WON BASS Teams - Entry Forms and Liability Release

You may mail your entry form to the regional director prior to the tournament or bring a printed copy to the tournament. The liability release is on page two of the entry form.  You must print out the liability release, sign it, and bring it with you to each event. 

You will need to have Adobe Acrobat Reader 7.0 or higher installed on your computer to type directly in to the entry forms and liability release online.  If you have a version prior to 7.0, you will need to print the form, then fill it out by hand.
